Summary
Job Summary
Responsible for oversight, planning, development, implementation, and evaluation of programs promoting evidence-based care of older adults.
Job Responsibilities
- Functions as a leader in the care of older adults by sharing expertise with nursing staff and members of the interprofessional team.
- Provides day-to-day support in the implementation of the programs supporting the care of older adults and related projects identified for the improvement of care.
- Introduces evidence-based clinical practice protocols and techniques that promote collaboration.
- Acts as the primary recipient and distributor of information from agencies supporting older adults and aging initiatives.
- Mobilizes, coaches, and mentors the committee members supporting the care of older adults.
- Creates exchange of learning opportunities for interprofessional team members across the organization.
- Develops and implements educational programming and activities focused on the care of older adults.
- Serves as an Owensboro Health Regional Hospital (OHRH) liaison for the care of older adults in the community.
- Contributes to the design, development, and implementation of age-related resources for patient, families and/or caregivers.
- Assists nursing leaders in the development, implementation and maintenance of a Geriatric Resource Nurse model for Owensboro Health Regional Hospital.
- Identifies and conducts nursing and interprofessional audit and research activities relevant to the area of practice.
- Performs job duties in accordance with the Nurse Practice Act.
Qualifications
- Bachelor's degree or higher in Nursing required upon hire
- A minimum of 3 years' relevant experience required
- RN - Licensed as a Registered Nurse. Eligible to practice nursing in the State of Kentucky required upon hire AND
- GRN-Geriatric Resource Nurse (GRN) through Nurses Improving Care for Healthsystem Elders (NICHE) required within 6 months of employment in this position AND
- GERON-Gerontological Nursing Certification through American Nurses Credentialing Center (ANCC) required within 18 months of employment in this position OR
- AGPCNP-BC - Adult-Gerontology Primary Care Nurse Practitioner-Board Certified through American Nurses Credentialing Center (ANCC) required within 18 months of employment in this position OR
- Physician Asst - Current licensure as a Physician Assistant in the state of Kentucky required within 18 months of employment in this position.
