Summary

Job Summary

Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
 
Job Responsibilities
  • Interviews patients, assesses priorities, and writes reports while managing complex caseload.
  • Completes comprehensive assessments of patients.
  • Assists patients/families with referrals to community agencies and provides education and resource information.
  • Works in collaboration with the interdisciplinary team to ensure quality support of care and safety of discharge.
  • Collaborates with leadership on complex discharge planning issues and delays in service.
  • Provides timely and concise documentation in the medical record, the assessment, concerns, needs, and discharge plan.
  • Keeps plans up to date as patient progresses toward discharge.
  • Supervises and/or trains other new employees, students, or interns as needed.
Qualifications
  • Bachelor's degree or higher in Social Work required upon hire
  • No experience required
  • LSW - Licensed Social Worker in the State of Kentucky required upon hire OR
  • LCSW - Licensed Clinical Social Worker in the State of Kentucky required upon hire OR
  • CSW - Certified Social Worker in the State of Kentucky required upon hire
